PERACare ID Card Reminders for 2026
The list below outlines which PERACare carriers will provide 2026 ID cards for the PERACare plans.
Pre-Medicare
- UMR Select CO – Only new enrollees will receive an ID card
- UMR National PPO – Only new enrollees will receive an ID card
- Kaiser EDCP – All enrollees will receive an ID card
- Kaiser DEDHMO – All enrollees will receive an ID card
Medicare
- UnitedHealthcare MA #1 – All enrollees will receive an ID card
- UnitedHealthcare MA #2 – All enrollees will receive an ID card
- Kaiser Med HMO – All enrollees will receive an ID card
Dental/Vision
No PERACare carriers for Dental and Vision plans will provide ID cards.
Getting Your ID Card
If you have not received a new ID card (as noted above) by December 31, 2025, please call the carrier to request a new one. Your provider can verify coverage through your carrier’s number until you receive your card. PERACare Premiums Reminder
New 2026 PERACare premiums will be deducted from your December 31, 2025, benefit check. Premiums for most plans are increasing for 2026, so be sure to plan accordingly. If you aren’t sure of your premium for 2026, log in to your secure account to view your monthly premium cost for the coverage.
