Information About Your 1099-R
Each year, Colorado PERA provides benefit
recipients with personalized year-end tax information. This
information is provided on IRS Form 1099-R, which is mailed
to each PERA benefit recipient by January 31.
Payments Reflect Benefit Amounts Paid in 2007 Tax
Year
The 2007 1099-R reflects benefit payments
issued from January 1, 2007, through December 31, 2007.
1099-R Information Available Online
PERA benefit recipients (service and
disability retirees and survivor benefit recipients) can view and print 1099-R information from the PERA Web site
using a PERA PIN. By
logging in to your PERA account with your PIN, you may
access and print 1099-R information from 2003 through 2007.
(The 1099-R option is available under the Inquiry drop-down
menu.)
Need a PERA PIN? (You'll receive your PIN within 7-10
business days after your request.)
Understanding Your 1099-R
This fact sheet explains information that may be printed
on a 1099-R for a PERA benefit recipient.
Instructions for Recipient
The IRS instructions that appear on the reverse side of
the 1099-R. To increase the font size while viewing this
document, click the magnifying glass icon with a plus
sign then click on the opened PDF document. (Use the
scroll bars located at the bottom and side of your
screen to move the document within the screen.) To print
this document with enlarged fonts, select Fit to Printer
Margins from the drop-down menu next to Page Scaling in
the Print window.
Requesting a Duplicate 1099-R
Service or Disability Retiree/Survivor
Benefit Recipient
You may request a duplicate 1099-R by
calling PERA's Customer Service Center at 1-800-759-7372 or
303-832-9550. Duplicates are printed overnight and mailed
daily from February 1 through April 15. You may also view and print your 1099-R for
calendar years 2003 through 2007 by logging in to the
secured pages of the PERA Web site with your PERA PIN.
Refund 1099
You may request a duplicate refund 1099-R by
calling PERA's Customer Service Center at 1-800-759-7372 or
303-832-9550. Duplicates are printed on Mondays and
Wednesdays and mailed
on Tuesdays and Thursdays.
401(k) Plan/DC Choice Plan
Call 1-800-759-7372 and select the 401(k)
option.
Short-Term Disability (STD)
The Short-Term Disability (STD) 1099-R is
issued by Standard Insurance. You may request a duplicate
1099-R by calling Standard Insurance at 1-800-368-1135.
Change of Address/Incorrect Address
Your 1099-R is still valid even if your
address on the form is incorrect. Please notify PERA of any address
change using one of the following methods:

Change your address online by using
your PERA PIN and logging in to the secured Account
Access section and click on Update Contact Information.
(If a PERA benefit check is mailed to your address, you
must submit your address change in writing and include
your signature.) |

information by mailing or faxing it to PERA, or calling
the PERA Customer Service Center. |
